Section 1: General Liability & Assumption of Risk

GENERAL LIABILITY WAIVER AND MEDICAL CONSENT

I AGREE:

• To allow the minor listed above to attend the CISAITSS Events LLC gathering.
• That participation in multiactivity park events (including but not limited to trampolines, skating, arcade games, and physical activities) carries inherent risks, and I assume full responsibility for any bodily injury, illness, or medical incident that may occur.
• To fully release, indemnify, and hold harmless CISAITSS Events LLC, its directors, staff, chaperones, and affiliated venue partners from any and all claims, demands, damages, or causes of action arising from my child's participation.

I UNDERSTAND THAT:

• I am aware that no facility or structured event can guarantee complete safety from injury.
• CISAITSS Events LLC is not responsible for safeguarding personal property, and attendees should leave valuables at home.
• In the event of a medical emergency, CISAITSS Events staff may contact emergency services, and I agree to be financially responsible for any medical care, treatment, or transport required.
• This consent is valid for the specific event date listed on this form.

Section 2: Media & Recording Consent

MEDIA AND RECORDING CONSENT

I agree that CISAITSS Events LLC may take my child's picture or record video during the event for identification, security, and promotional purposes.

I waive any right to inspect or approve the finished product and waive any right to royalties or compensation for the use of these images or recordings.

This may be used on social media.

EVENT TERMS, CONDITIONS, AND BEHAVIOR PROTOCOLS

1. GENERAL EXPECTATIONS & SUPERVISION

• While adult chaperones and venue staff will be present for the duration of the event to ensure a structured environment, each attendee and their guardian are solely responsible for the attendee's actions, behavior, and adherence to these Terms and Conditions.

2. ZERO-TOLERANCE BEHAVIOR POLICY

• Disrespectful, disruptive, or inappropriate behavior will not be tolerated under any circumstances.
• There is a strict, zero-tolerance policy for the following infractions:
• Physical altercations, aggressive behavior, or bullying of any kind.
• Destruction, theft, or vandalism of venue property or CISAITSS equipment.
• Disrespect or insubordination toward chaperones, venue staff, or other attendees.
• Entry into restricted venue zones or unauthorized exits.

3. LAW ENFORCEMENT & INCIDENT PROTOCOL

• CISAITSS Events prioritizes the safety of all attendees.
• If any individual becomes violent, poses a threat to others, or causes severe disruptions during the event, the Michigan State Police will be dispatched and notified immediately without exception.
• Any serious threats of violence, verbal or physical, will be reported to law enforcement.

4. RIGHT TO REMOVAL

• CISAITSS Events and venue staff reserve the absolute right to revoke entry or remove any individual exhibiting behavior that violates these terms.
• If an attendee is removed, parents/guardians will be contacted immediately and are required to pick up the attendee from the venue promptly.
• Removed individuals will not be allowed to return to the premises.

5. TICKET, PAYMENT, AND REFUND POLICY

• All ticket sales are final.
• No refunds will be issued for no-shows or removals due to behavior violations.
• I authorize any responsible payment method to pay CISAITSS Events LLC for the purchased ticket.
• I understand that any attempt to initiate a fraudulent bank or credit card chargeback for a validly purchased ticket (or following a behavioral removal) will result in a permanent ban from all future CISAITSS Events.

6. COMPLIANCE & AMENDMENTS

• By attending this event, you acknowledge that you have read and agreed to these Terms and Conditions.
• Failure to comply will result in removal, potential law enforcement notification, and a permanent ban from future gatherings.
• CISAITSS Events LLC reserves the right to modify or update these terms as necessary.
