Please read and agree to the following:

I have read and fully understand the expectations and responsibilities associated with my child's selection as a MiniPerformer with Dance Theatre South (DTS). I recognize that participation in this program is a privilege and requires a commitment to the standards and schedule set forth by DTS. I acknowledge that all rehearsals, fees, and performances are completely separate from Elevate Dance & Fitness Studio.

I understand and agree to the following:

• My dancer is required to attend at least one ballet class per week.
• My dancer is expected to attend all scheduled rehearsals in preparation for the production.
• Prior notice is required if my dancer must miss a scheduled rehearsal.
• My dancer's agreement form and performance fee of $150 is due September 10th
• Performance fee includes: Cast Party, Show T-shirt, and Show Video
• (Accepted payment methods: cash, check payable to DTS, or Venmo: @DTSdance)

I am committed to supporting DTS's mission of cultivating dedicated and disciplined dancers. I will ensure that my child arrives at class and rehearsal properly dressed, with appropriate hair and attire. I will teach my child to demonstrate respect and appreciation for instructors, directors, and fellow dancers.

I agree to serve as a positive representative of DTS in the community and on social media. I understand that all fees are non-refundable, and I accept the terms of this agreement in full. I further agree to comply with, and support my dancer in complying with, all program expectations.
