Swings4Theo 3rd Annual Wiffle Ball Registration

Join us on Saturday, September 12, 2026 at Georgetown Little League in Fort Wayne, IN for the 3rd Annual Wiffle Ball Games. This year registrations will be accepted as individual players or teams of up to six players. For any individual players, teams will be created at the event. This is a day designed to focus on having fun, a day where the little kids can play with the big kids (old kids are welcome, too), and all skill levels and abilities are welcome! This donation provides a free registration for a kid with medical challenges to participate in the wiffle ball game. Any additional funds will support the Swings4Theo organization.

Liability Release and Waiver of Claims

In consideration of the opportunity for my child to participate in the Swings4Theo Annual Wiffle Ball Game (the “Event”), I, the undersigned, being the parent or legal guardian of the participant, hereby agree to the following on behalf of myself and my child:

To the fullest extent permitted by Indiana law, I hereby release, waive, discharge, and covenant not to sue Swings4Theo, their directors, officers, volunteers, employees, agents, affiliates, sponsors, and all other associated personnel or entities (collectively, the “Released Parties”) from any and all claims, demands, actions, or causes of action for injury, loss, damage or death, including but not limited to bodily injury, personal injury, property damage, or wrongful death arising out of or related to participation in the Event whether or not caused by the negligence of the Released Parties.

I hereby grant permission to Swings4theo or their volunteer photographers to use my image, voice, and likeness in photographs, videos, and other digital media on their social media platforms, including but not limited to Facebook, Instagram, Twitter. Swings4Theo is authorized to use this media for promotional and marketing purposes related to their organization. I understand that I will not receive any compensation for this use. I waive any rights of compensation or ownership to the media created. By signing below, I acknowledge that I have read and understand the terms of this release. I understand I may opt out of this provision by providing written notice before the Event.
