ASSUMPTION OF RISK, RELEASE OF LIABILITY, MEDICAL CONSENT, AND MEDIA RELEASE
By submitting this registration form, I, the participant, or the parent/legal guardian of the participant if the participant is under 18 years of age, acknowledge and agree to the following:
ASSUMPTION OF RISK AND RELEASE OF LIABILITY
I understand that participation in soccer activities, training sessions, camps, combines, showcases, competitions, travel, and related activities involves inherent risks, including but not limited to serious bodily injury, concussion, permanent disability, paralysis, illness, property damage, and death. I understand that these risks may result from my own actions or inactions, the actions or inactions of others, the condition of facilities or equipment, weather conditions, or other known and unknown factors.
I voluntarily assume all risks associated with participation in any program, event, camp, combine, showcase, training session, or activity conducted by Grove Soccer Academy LLC.
In consideration for being permitted to participate, I hereby release, waive, discharge, covenant not to sue, and hold harmless Grove Soccer Academy LLC, its owners, officers, directors, employees, coaches, trainers, volunteers, agents, representatives, sponsors, affiliated organizations, event partners, facility operators, and property owners (collectively referred to as the "Released Parties") from any and all claims, liabilities, demands, damages, losses, costs, expenses, or causes of action arising out of or relating to participation in any Grove Soccer Academy LLC activity, including transportation to and from such activities, to the fullest extent permitted by law.
MEDICAL CONSENT
I authorize Grove Soccer Academy LLC and its representatives to obtain emergency medical treatment for the participant if deemed necessary. I understand that every reasonable effort will be made to contact a parent, guardian, or emergency contact before treatment is administered when circumstances permit.
I accept full financial responsibility for any medical treatment, transportation, emergency services, hospitalization, or related expenses incurred on behalf of the participant.
I further release and hold harmless the Released Parties from any liability arising from the provision of emergency medical care, except as prohibited by applicable law.
CONCUSSION AND RETURN-TO-PLAY POLICY
Participant safety is a top priority. Grove Soccer Academy LLC reserves the right to remove any participant from training, competition, or camp activities if signs or symptoms of injury, illness, concussion, or other medical concerns are observed.
If a participant is diagnosed with, or suspected of having, a concussion or other traumatic brain injury (TBI), the participant will not be permitted to return to participation until Grove Soccer Academy LLC receives written clearance from a licensed physician or qualified medical professional authorized to provide such clearance.
The decision of Grove Soccer Academy LLC regarding a participant's removal from activity for safety reasons shall be final.
PHOTO AND VIDEO RELEASE
I grant permission to Grove Soccer Academy LLC and its authorized representatives to photograph, film, record, and otherwise capture the participant's likeness, image, voice, and performance during any camp, event, training session, or activity.
I authorize Grove Soccer Academy LLC to use, reproduce, publish, distribute, broadcast, display, and otherwise utilize such photographs, videos, recordings, and related materials for promotional, educational, marketing, social media, website, and other lawful business purposes without compensation, notice, or further consent.
ACKNOWLEDGEMENT
I certify that I have carefully read this Agreement, fully understand its contents, and understand that I am giving up certain legal rights by agreeing to it. I acknowledge that I am signing this Agreement voluntarily and intend for it to be a complete and unconditional release of liability to the fullest extent permitted by law.
