Statement of Faith: The Wilderness Canoe Trip is a ministry of the Off Lake Bible Church. Our goal is to encounter God through adventure in His creation, Bible teaching, small group discussion, and alone time with God. We encourage campers to disconnect from the noise of our world and connect and grow in their relationship with God alongside friends. We believe the Bible is the Word of God, the only rule for beliefs and life. We believe in the Trinity — God the Father, God the Son and God the Holy Spirit. We believe that Jesus Christ died on the cross for our sins and rose again, giving us assurance of everlasting life when we place our faith and trust in Him. On our canoe trip, youth are invited to know Jesus Christ deeper through Bible teaching, scripture, and discipleship.
Banned/illicit Substances: Wilderness Canoe Trip is a youth based ministry, and as such we adhere to a strict no-tolerance policy in regards to illicit substances, dangerous weapons, or other deemed hazards. Illicit substances are strictly forbidden and as such, we reserve the right to confiscate any substance that we deem to fall under this threshold. This includes but is not limited to, Alcohol, tobacco products, smoke packs, nicotine vapes, non nicotine vapes, marijuana products, weapons, firecrackers, etc. Failure to adhere to our policy may result in camper being sent home early with no refund. By registering your child for this canoe trip, you are hereby agreeing to our policy.
Electronic Device Policy: Electronic devices of any kind (ie. Cellphone, Tablet, Handheld Video Game, etc.) are prohibited on the Wilderness Canoe Trip with the exception of standalone digital cameras. Any such devices brought on the trip will be confiscated on discovery for the duration of the trip.
Discipline Policy: The desire of the Wilderness Canoe Trip is for campers, staff and volunteers to participate fully in the camp program. In an effort to maintain consistent application of the camp code of conduct and camp rules a "three strikes and you're out" system will be implemented. Warnings are allowed but are not mandatory. Additionally, if, at the discretion of the camp director, the actions of any camper, staff,volunteer or guest become dangerous to themselves or others, disruptive to the camp program or disturbing to other campers, staff or volunteers, expulsion from the program and property is warranted without warning or "strikes." In all cases where expulsion is prescribed, parents and/or guardians assume full responsibility and expense for retrieving their camper from the trip, even if the trip is held in a remote location. Likewise, staff and volunteers bear their own responsibility and expense for their dismissal from camp programs.The camp organization will charge a fee equal to 100% of expenses it incurs as a result of expulsions and dismissals.
Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Off Lake Bible Church during the Wilderness Canoe Trip. In exchange for the acceptance of said child’s candidacy by Off Lake Bible Church, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Off Lake Bible Church and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected Wilderness Canoeing sessions.

In case of injury to said child, I hereby waive all claims against Off Lake Bible Church including all directors, counselling staff, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including canoeing. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.
Medical Release and Authorization

As Parent and/or Guardian of the named camper, I hereby authorize the diagnosis and treatment of the minor child, in the event of a medical emergency, which in the opinion of the attending first aid responder, requires immediate attention to prevent further endangerment of the minor’s life, physical disfigurement, physical impairment, or other undue pain, suffering or discomfort, if delayed.

Permission is hereby granted to the Wilderness Canoe Trip First Aid Responder to proceed with any medical or First Aid treatment for the named camper. In the event of an emergency arising out of serious illness, the need for major surgery, or significant accidental injury, I understand that every attempt will be made by the attending physician to contact me in the most expeditious way possible. This authorization is granted only after a reasonable effort has been made to reach me.

Permission is also granted to Off Lake Bible Church and its affiliates including Directors, Camp Counselors, and Support staff to provide the needed emergency treatment prior to the child’s admission to the medical facility.

Release authorized on the dates and/or duration of the Wilderness Canoe Trip.

This release is authorized and executed of my own free will, with the sole purpose of authorizing medical treatment under emergency circumstances, for the protection of life and limb of the named minor child, in my absence.
I authorize designated leaders on the trip to administer appropriate doses (as per the recommendation of the product label) of the following over-the-counter medication to my child when deemed necessary
