Youth Participation Waiver and Release of Liability
By purchasing or renewing a membership with West Valley Homeschool Foundation, I acknowledge and agree to the following on behalf of myself, my child or children, and any minor participants for whom I am the parent or legal guardian:
1. Responsibility to Check for Safety
I understand that West Valley Homeschool Foundation activities may take place in a variety of locations, including homes, parks, rented facilities, public spaces, and other event locations.
I agree that it is my responsibility as the parent or legal guardian to inspect the area, equipment, and activity before my child participates. If I believe anything is unsafe, I will immediately notify the event organizer or supervising adult and will not allow my child to participate until the concern is addressed.
2. Assumption of Risk
I understand that participation in West Valley Homeschool Foundation events and activities may involve risks. These risks may include, but are not limited to, falls, collisions, illness, injury, property damage, or other unexpected harm.
I understand that injuries may be minor or serious, and in rare cases may include permanent disability or death. I also understand that risks may result from my child’s actions, the actions of other participants, the actions or inactions of adults present, the condition of the location, the use of equipment, or other circumstances that may not be known or foreseeable at the time.
By choosing to participate, I voluntarily accept these risks.
3. Parent/Guardian Responsibility
I understand that West Valley Homeschool Foundation is a homeschool community organization and that parents and legal guardians remain responsible for the supervision, safety, behavior, and participation of their own children unless a specific event states otherwise in writing.
I agree to be responsible for any injury, damage, loss, or expense that may result from my child’s participation, including damage caused by my child to people, property, facilities, or equipment.
4. Release of Liability
To the fullest extent allowed by law, I release and agree not to sue West Valley Homeschool Foundation, its leaders, board members, volunteers, organizers, members, participating families, sponsors, advertisers, affiliated groups, and any owners or lessors of locations used for events or activities.
This release applies to any claims, demands, losses, injuries, damages, or expenses related to participation in West Valley Homeschool Foundation events or activities, including claims involving injury, death, or property damage, whether caused by negligence or otherwise, to the extent permitted by law.
5. Membership Acknowledgment
I understand that this waiver and release of liability is a requirement of membership in West Valley Homeschool Foundation.
By submitting payment for membership, I confirm that I have read this waiver, understand it, and acknowledge that I am giving up certain legal rights on behalf of myself and my participating minor child or children.
Adult Participation Waiver and Release of Liability
By purchasing or renewing a membership with West Valley Homeschool Foundation, I acknowledge and agree to the following for myself as an adult member or participant:
