Health Innovation Community Partnership (HICP) Voting Membership Interest Form

HICP members are encouraged to:

• Participate in Partnership meetings and events
• Share information and resources with their communities
• Support community-driven solutions and engagement
• Promote collaboration and equitable opportunities for Eastside residents
• Help elevate community voices in planning and decision-making processes

1. Attend and actively participate in all Partnership meetings (permitted to miss three per year).
2. Serve on at least one Committee (either Executive Committee or non-Executive Committee/working group).
3. Promote Partnership values, perspectives, and activities to external audiences.
4. Help guide and develop the Partnership.
5. Meaningfully engage around community voices and interests.
6. Work to secure community benefits for current and future residents.
7. Allow your name, title, logo, and likeness to be used in promotional materials of the Partnership.
