Safety & Welfare

All Children undertaking any football coaching are advised to wear approriate clothing, footwear, and shinguards. Any child attending the football sessions deemed not to be wearing the suitable equipment may not be permitted to take part.

Please make sure all children bring a packed lunch/ substinence for the duration of the day, and a drink. Please bring a change of indoor shoes and a rain jacket, in case of bad weather.

CHILDREN WILL NOT BE ALLOWED TO LEAVE THE GROUNDS ON THEIR OWN AT ANY TIME
Confidentiality

Details on this form will only be kept until the course has been completed and will only be shared with coaches or others who may need this information to meet the specific needs of your child. It will not be shared elsewhere, abused or sold on.
Data Protection

I understand that Selsey FC may store sensitive personal information about myself and my child including medical conditions and very personl information. I give my consent for the details I submit on this form to be kept on file inline with Selsey FC Insurance policy.
Electronic Signature

An electronic signature is as contractually binding as a written paper one, please ensure that you have read all the form and submitted all the required answers.
I hereby give my approval for my childs participation in Selsey FC Summer Football Camp. In exchange for the acceptance of said child's booking I accept all risks and hazzards incidental to the conduct of the activities, and release, absolve and hold harmless Selsey FC and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of travelling to, participating in, or returning from selected camp sessions. Incase of injury to said child, I hereby waive all clalims against Selsey FC including all coaches and affilliates, all participants, and, if applicable, owners and lessors of premises used to conduct the event.

There is a risk of being injured that is inherent in all sports activities including football.
