The Encore is our grand finale to summer camp, bringing together some of our favorite Project Play experiences in one unforgettable week. Get ready for popsicles on the lawn, foam parties, water play and all the messy fun!

Authorization
- I, the parent/guardian of this camper gives authority to the staff of this camp to apply judgment in regards to medical assistance in the event of an accident, injury, or illness if the emergency contact person cannot be reached. I authorized first aid, medical/surgical diagnosis, and treatment which may deem necessary.
- I released the organizers, staff or Project Play Professional Corporation for any responsibility in case of accident, illness, or injury during my child's enrollment.
- I confirm that all information given in this form is true, complete, and accurate.

In consideration of my child being permitted to enter the play area and participate in any programs, camps, parties, or field trips at Project Play Professional Corporation, I certify that I am the parent or legal guardian of the participant, or authorized to sign on their behalf.

I agree to follow all posted rules, safety guidelines, and staff instructions, and understand that I am responsible for my child’s safety and behavior at all times. Any unsafe condition will be reported to staff immediately.

I acknowledge that participation involves inherent risks, known and unknown, including those caused by other participants. On behalf of myself and my child, I voluntarily assume all such risks.

I release and hold harmless Project Play Professional Corporation, Project Play Discovery Museum, and their owners, employees, agents, affiliates, and representatives from any and all claims, injuries, damages, or liabilities arising from participation in activities or use of the facility.

I understand that the images may be used in print and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.

Our primary goal at Project Play is to provide a safe, positive, and enjoyable environment for all participants. By signing below you understand that Project Play staff reserves the right to dismiss your child from camp if your child's behavior poses a threat to themselves or others at any point during our weekly camps. No refunds will be provided in such cases.

Cancellation Policy

If you withdraw from a camp seven calendar days or more from the camp start date you will have the following options: Option 1: Request a Program Credit. Credit the customer’s account with the balance for a future class, camp, special event, or program. Credits must be used within one year. Option 2: Transfer camp dates to another available date. We do not give refunds for illness or absences after the seven day period. Option 3: Request a 50% Camp Refund
