Community Soccer Registration

Liability Waiver, Assumption of Risk & Release Section:

I, the parent/legal guardian of the registered participant, acknowledge and understand that participation in soccer, physical activity, training, drills, games, 5-on-5 play, running, jumping, kicking, indoor gym activities, outdoor field activities, and all related Community Soccer activities involves inherent risks.

These risks may include, but are not limited to, slips, trips, falls, collisions with other participants, contact with balls, equipment, walls, gym floors, field surfaces, weather-related risks, muscle strains, sprains, bruises, cuts, fractures, dental injuries, concussions, illness, medical emergencies, or other injuries.

I understand that while reasonable efforts will be made to provide a safe, organized, and supervised environment, risks cannot be completely eliminated.

I voluntarily give permission for my child to participate in Community Soccer programming.

I confirm that my child is physically able to participate unless otherwise noted in the medical section of this form.

I agree to release, waive, discharge, and hold harmless My Community Soccer, Whalen Hockey Training, its owners, directors, coaches, instructors, volunteers, staff, representatives, independent contractors, agents, and affiliates from any and all claims, demands, actions, damages, costs, losses, expenses, or liability arising out of or related to my child’s participation in Community Soccer programming.

I further agree to release, waive, discharge, and hold harmless the Grand Erie District School Board, its trustees, employees, representatives, volunteers, agents, facility staff, and any school or facility used for the program from any and all claims, demands, actions, damages, costs, losses, expenses, or liability arising out of or related to my child’s participation in Community Soccer programming, including the use of school facilities, gyms, fields, parking lots, entrances, exits, washrooms, and related areas.

I understand that My Community Soccer and Whalen Hockey Training are independent organizations and are not operated by, sponsored by, or affiliated with the Grand Erie District School Board.

I agree that I am responsible for ensuring my child follows program rules, respects coaches, participants, volunteers, facilities, and equipment, and participates in a safe and respectful manner.

I understand that failure to follow safety rules or behaviour expectations may result in removal from the program without refund.

Cancellation and Refund Policy:

Registration is limited and spots are reserved once payment is received.

The $49 deposit is non-refundable.

If a participant withdraws before the program begins, any amount paid above the $49 deposit may be refunded.

Once the program has started, refunds will not be issued except in special circumstances approved by My Community Soccer.

Missed sessions due to illness, vacation, scheduling conflicts, weather, school closures, facility closures, or personal reasons are not refundable.

If a session is cancelled by My Community Soccer, reasonable efforts will be made to reschedule the session when possible.

If the facility becomes unavailable due to school board closure, weather, emergency, or circumstances outside of our control, My Community Soccer will make reasonable efforts to reschedule but refunds are not guaranteed.
