Certificate of Completion:
This certificate acknowledges that the individual named below has successfully completed the Community Inclusion, Integration, and Trauma-Informed Consent training course provided by Nobility Reigns.
This training provided comprehensive instruction on the principles of person-centered, trauma-informed, and strength-based care within Adult Family Homes. The course reviewed the legal and ethical foundations of community inclusion and integration, including the Americans with Disabilities Act and the importance of avoiding unnecessary segregation. Employees were trained on recognizing the impact of trauma and adverse childhood experiences, promoting emotional and physical safety, respecting resident autonomy, and supporting meaningful engagement both within the home and in the broader community.
The training also reinforced Nobility Reigns’ policy expectations regarding daily engagement and documentation. Employees were instructed on the requirement for first and second shift staff to offer and document a minimum of three activities per shift, per client, resulting in at least six activity offerings per day, and to document whether activities are accepted or declined. The course further addressed the role of third shift staff in supporting engagement, including offering life-skill activities such as meal preparation when appropriate. Emphasis was placed on accurate activity documentation as a critical component of person-centered care, regulatory compliance, and respect for client choice.
By completing this training, the employee acknowledges an understanding of their responsibility to support dignity, inclusion, choice, and meaningful participation for all individuals served by Nobility Reigns, and to follow all related policies and documentation requirements.
