* For the apparel, this logo will be a screen print that is smaller on one side of the breast of the front of the item, and a larger screen print of the logo will be centered on the back of the apparel. All apparel items ordered will have this logo on both the front and the back (not pictured in photos below).

BEFORE SUBMITTING THIS FORM:

1. Please be sure that you have selected your payment type above and calculated your total if sending payment via PayPal (a 3% convenience fee will be appled to your order total).

2. Ensure that you have submitted your payment (if paying via Zelle or PayPal, include your Zelle Transaction ID or PayPal Transaction ID) prior to submitting your pre-order. If you are paying via check, please be sure to include your check #.

3. Once you submit your pre-order, you will receive an automated email confirmation showing your pre-order selections. If you need to make changes to your order, please reply to your confirmation email detailing what changes are needed within 24 hours of submitting this pre-order form.
