Guest Waiver/ Release Form
GUEST RELEASE AND COVENANT
WHERAS, I (guest Name) acknowledge and understand that:
A. Firearms have an inherent risk;
B. Shooting sports and archery may be inherently risky and personal injury, bodily injury and /or property damage may occur in the course of shooting sports or archery;
C. Proper safety procedures may reduce the occurrence of such injury or damage arising from shooting sports or archery, but cannot eliminate such occurrences;
D. Shooting sports and archery do occur on the premises of the Lafayette Gun Club;
E. As a guest I will always obey the rules of Lafayette Rod & Gun Club and the directions of the member listed above.
NOW, THEREFORE I, for valuable consideration for which receipt is hereby acknowledged, including but not limited to as an invited guest access and use of the facilities of the Lafayette Rod & Gun Club, subject to the rules and regulations thereof, and participation in the activities of the Lafayette Rod & Gun Club, do hereby release the Lafayette Gun Club, its officers, directors, agents and servants from any and all claims, demands, action, judgments, and executions which may directly accrue to me or accrue to me by or through any other person or party, for any personal injury, bodily injury or property damage to any person or any property occurring on the property of the Lafayette Rod & Gun Club. I further specifically release any and all negligence claims against the Lafayette Rod & Gun Club, its officers, directors, agents and servants or employees. I will indemnify and hold harmless the Lafayette Rod & Gun Club, its officers, director, agents and employees for any injury to any guest of mine or damage to the property of any guest of mine, where such injury or damage occurs on the property of the Lafayette Rod & Gun Club. Nothing in this Release shall apply to any injury caused by the intentional action of the Lafayette Rod & Gun Club or its officers, directors, agents or servants or any injury caused by an act where such an injury was wantonly and willfully caused by the Lafayette Rod & Gun Club or its officers, directors, agents, servants or employees. This Release does not release any claims against individual members of the Lafayette Rod & Gun Club or their agents for actions taken by such individual members, or their guest, on the premises of the Lafayette Rod & Gun Club. However, this Release does release the Lafayette Rod & Gun Club and its officers for any injury or damage caused by an act where such injury was wantonly and willfully caused by a member of the Lafayette Rod & Gun Club or for any negligent act of any member or their guest on the premises of the Lafayette Rod & Gun Club.
