Waiver and Release of Liability
I acknowledge that participation in Fort Saskatchewan Praise, organized by RCCG, Place of Rest Parish, involves outdoor activities that may carry inherent risks. By attending, I voluntarily assume all risks associated with participation and hereby release RCCG, Place of Rest Parish, its organizers, volunteers, and affiliates from any and all liability for injury, loss, or damage to person or property.
Indemnity Agreement
I agree to indemnify and hold harmless RCCG, Place of Rest Parish, its organizers, volunteers, and affiliates from any claims, demands, or causes of action arising out of my participation in this event.
Medical Authorization
I authorize event organizers to secure medical care for myself or my child in the event of an emergency, and I accept responsibility for any related costs.
Parent/Guardian Consent for Minors
If I am completing this form for a minor, I confirm that I am the parent or legal guardian and have the authority to accept these terms on the minor’s behalf.
Media Release
I grant permission for photographs, video, or audio recordings taken at the event to be used for promotional or informational purposes by RCCG, Place of Rest Parish, without compensation.
