  • BRIDGE is designed to support your family with the day‑to‑day parts of Type 1 diabetes — the real‑life moments that happen between clinic appointments. Support is available during business hours (Mon–Fri), and you can reach out in the way that suits you: text, email, phone or telehealth.

    For urgent or after‑hours concerns, please use your usual medical and emergency pathways (GP, diabetes team, Nurse‑on‑Call, or 000).

  • To make sure BRIDGE is the right fit for your family, please confirm the following:*
  • Program Requirements

    BRIDGE appointments are bulk‑billed through Medicare using a GP Chronic Condition Management Plan (GPCCMP) with visits allocated to diabetes education. (If you’re not sure whether you have one, we can help you check.)

    BRIDGE also has a simple monthly fee of $25 per month during the pilot phase. This helps cover the non‑billable support families rely on.

  • Once you submit this form, we will add your details to our clinical system (Cliniko). You’ll then receive a set of forms by email — these include your clinical consent, privacy information, and a few short questionnaires to help us understand your family’s needs.

    After that, we will contact you within 1–2 business days to organise your BRIDGE Start Session, where we’ll talk through your child’s diabetes, your routines, your goals, and how BRIDGE can support your family over the next 12 weeks.
