By signing up for this 4 week- AI Future Builders Certificate Program, the student is committed to attending every single designated day set by the program. If you miss more than 2 days, you will be dismissed from the program.
Hold Harmless Agreement: I, as the parent/guardian, agree to hold harmless Onyx Foundation, Inc., and its representatives from any liability or claim arising from participation in the AI Future Builders Certificate Program.
Onyx Foundation, Inc. Media and Student Work Release Waiver: I, grant permission to Onyx Foundation, Inc., its employees, volunteers, partners, sponsors, and authorized representatives to photograph, video record, audio record, and use the name, image, likeness, voice, and/or student-created work of me and/or my minor child while participating in any Onyx Foundation program, activity, or event.I understand these materials, including presentations, artwork, business concepts, written assignments, digital projects, and AI-generated or AI-assisted work created through program participation, may be edited, reproduced, published, displayed, and distributed worldwide for lawful purposes, including educational, promotional, fundraising, grant reporting, social media, website, print, and other electronic communications. Such materials may be used with or without identifying names. I waive the right to inspect or approve the final use of these materials and understand that neither I nor my minor child will receive compensation for their use. I release and hold harmless Onyx Foundation, Inc., its directors, officers, employees, volunteers, partners, sponsors, and affiliates from any claims arising from the use or publication of these materials, including claims related to invasion of privacy, defamation, false light, or misappropriation of name, image, likeness, voice, or creative work. This authorization is voluntary and irrevocable. By signing below, I acknowledge that I have read and understand this Release and voluntarily agree to its terms. If signing on behalf of a minor child, I certify that I am the child's parent or legal guardian and have the authority to grant this permission on the child's behalf.
