Epic Allstars Liability Waiver: By allowing my child(ren) to participate in an EPIC Allstars event with Coach Van, I understand and acknowledge the inherent risks associated with physical activities in a cheerleading/gymnastics environment. I hereby release and hold harmless EPIC Allstars and Coach Van, its owners, directors, officers, employees, coaches, and agents from any and all liability for injuries or damages that may occur to myself, my child(ren), or accompanying family members during participation in these events, including during any supervised or unsupervised time, and including travel to and from the facility. I further agree to indemnify and hold harmless the above-mentioned parties against any and all claims, actions, damages, costs, or expenses arising from or in connection with any injury or property damage incurred by me, my child(ren), or any accompanying individual during participation.

Epic Allstars Medical Waiver: I certify that I am the parent or legal guardian of the child(ren) named and that they are in good physical health and able to safely participate in physical activities. In the event of an accident, injury, or illness, I authorize EPIC Allstars staff or Coach Van to seek and administer appropriate medical care. I agree to be solely responsible for any costs associated with such care. I understand that by signing this waiver, I release EPIC Allstars and Coach Van from any liability related to the medical care or emergency services rendered.

Epic Allstars Media Waiver: I grant EPIC Allstars and Coach Van permission to take and use photographs and/or video of my child(ren) taken during tumble clinic sessions for marketing, promotional, or social media purposes. I understand that no compensation or royalty will be due to me now or in the future for the use of these materials.
