Please sign below that you adhere to the following policies and waiver:
- DEADLINE: Forms must be submitted no later than Friday, September 18, 2026.
- Event Setup is between 8:00am – 9:00am DST.
- All vehicles must park in designated area.
- We encourage all our booth participants to stay for the entire event, from 8:00am-2:00PM DST.
- There will be no electrical support for booths.
- Bring your own tables and chairs, and canopy

I do hereby release and discharge Tuba City Regional Health Care Corporation, its officials, officers, volunteers and employees from any and all claims from injuries, damage or loss, which I may have, or which may occur to me as a result of my participation or attendance in any of TCRHCC program, activities, and/or events.
