What workshops are you booking for?

Does your child suffer from any injuries, illness, disabilities, allergies or any other medical conditions?

Students will be required to pack lunches/snacks, and some of the activities will involve food, so please list any allergies so we can request they not be brought into the studio

I understand this workshop comes with a cost of $50 and must be paid on arrival. A 50% cancellation fee may apply if a cancellation is made within 24 hours of the workshop start time. Full payment must be made if cancelled within 1 hour of the start time or if the student is a no show.

I give permission for Hype Dance HV to take and use photos/vidoes of my child. I understand these images may appear on social media and other media e.g. newspapers.

Liability Waiver

I am not aware of any injury, illness or other health related issues that would restrict or limit my child's ability to participate in this workshop.

I agree to assume all risks and expenses if an injury is to occur from participation in this workshop.

I agree to hold Hype Dance HV or anyone acting on it behalf either as a teacher, assistant teacher or administrator harmless in the event of an injury to my child while participating under the supervision of Hype Dance HV.
