LifeCare Health Care Fund Community Projects Grant Application

The LifeCare Health Care Fund, a fund of the Northwest Minnesota Foundation, was established to provide financial support to qualified nonprofit organizations for educational, charitable, and health-related activities, projects, and services that benefit residents and visitors of Roseau County.
Application Process

Once completed, the application will be submitted to the LifeCare Health Care Fund Advisory Committee for review. A personal presentation explaining your grant proposal may also be requested. This form will be submitted electronically to Tanya Semingson, Development Coordinator at LifeCare Medical Center. Applications are due no later than 4:30 PM on August 27, 2026.
Grant Proposal Guidelines for Community Projects

The LifeCare Health Care Fund will consider grant requests from nonprofit organizations for monies from the following funds:

LifeCare Health Care Fund General Fund - The purpose of this fund is to provide financial support for health care, wellness, and senior living related charitable and educational activities, projects or services benefiting residents and visitors to the LifeCare Medical Center service area.

Selection Criteria will focus on:
- How the project/program fits within the mission of the LifeCare Health Care Fund, as well as the purpose of the fund being accessed.
- The scope of the project/program.
- The number of participants served, number of communities served and number of organizations served.
- Please note that funding is for future projects only; reimbursements are not eligible.
- Funding is only for nonprofit organizations with current IRS tax-exempt status or public, governmental organizations such as a township, city, county or school district.
Evaluation

A written evaluation or summary of the impact of the program or item funded through the grant will be requested at the end of the grant period.
