TESTIMONY RELEASE & MEDIA CONSENT AGREEMENT
SENORITAS WITH CRISTO MINISTRIES

By submitting this form, you acknowledge and agree that your testimony, interview, and/or statements may be recorded by SENORITAS WITH CRISTO (“Ministry”) in audio, video, written, or digital format.

You grant SENORITAS WITH CRISTO a non-exclusive, royalty-free, worldwide, perpetual, and irrevocable license to use, reproduce, edit, publish, distribute, and display your testimony for ministry purposes. This includes use across all current and future platforms, including but not limited to podcasts, social media, websites, printed materials, and promotional content.

You understand that your testimony may be edited for clarity, length, formatting, translation, or content organization, while maintaining the general intent of your message.

You acknowledge that participation is voluntary and that you will not receive financial compensation now or in the future for any use of your testimony.

You agree that this recording is not confidential and does not create a counseling, therapist-client, or attorney-client relationship.

You may withdraw consent within 7 days of recording by submitting written notice to SENORITAS WITH CRISTO at senoritaswithcristo@gmail.com.

After this period, you acknowledge that materials already published or distributed may not be removed.

To the fullest extent permitted by law, you release SENORITAS WITH CRISTO, its directors, volunteers, and representatives from liability arising from the use of your testimony as described in this agreement.
