I authorize The Lilypad Initiative LLC to receive and review a consumer report obtained through OneSource, Inc., an independent third-party consumer reporting agency, in connection with my application for guest membership on the Lily platform. I understand that I will complete and pay for my background screening directly through OneSource and that Lily will receive my results solely to evaluate my eligibility for platform membership. I have received and reviewed Lily's standalone Background Check Disclosure document prior to submitting this application.
