    NThis Enrollment Agreement is entered into between Pulse Institute of Allied Health ("Institution") and the Student identified on this Agreement. By signing this document, the Student agrees to pay all applicable tuition, fees, and charges associated with enrollment in the selected program and acknowledges receipt of the School Catalog, Student Handbook, institutional policies, and enrollment disclosures.

    Pulse Institute of Allied Health agrees to provide instruction, educational services, and program resources in accordance with applicable Arkansas Division of Higher Education (ADHE) regulations, institutional policies, and program requirements.


    PROGRAM OFFERINGS & TUITION DISCLOSURE


    Please select the program for which you are seeking enrollment. By selecting a program, you acknowledge that you have reviewed and understand the program length, delivery format, clock hours, tuition, fees, and total cost of attendance.


    Medical Assistant Program (MA)
    Program Length: 8 Weeks | Delivery Method: Online | Clock Hours: 120

    Phlebotomy Technician Program (PT)

    Enrollment Acknowledgment
    By submitting this enrollment application, I acknowledge and understand that:

    The tuition and fee amounts listed above represent the published program costs.
    Registration fees are non-refundable unless otherwise required by applicable law.
    Enrollment is not guaranteed until all admissions requirements have been satisfied.
    Tuition discounts, scholarships, payment plans, and promotional pricing may be offered separately and are not guaranteed.
    I am responsible for all tuition, fees, and educational costs associated with my selected program.
    I understand that completion of this application does not constitute acceptance into the program.
    My seat is not reserved until all required enrollment documentation and applicable payments have been received and approved by Pulse Institute of Allied Health.

    National Exam Fees are not included ranging from $130-$169



    PROGRAM DISCLOSURE


    Pulse Institute of Allied Health reserves the right to revise, update, substitute, or modify course content, educational materials, software platforms, textbooks, simulation resources, and instructional methodologies when necessary to maintain educational quality, regulatory compliance, or industry relevance. Such modifications shall not materially reduce the quality, educational objectives, or instructional length of the program.

    Students are responsible for maintaining satisfactory academic progress, meeting attendance requirements, completing all assignments, and complying with institutional policies.

    Academic dismissal may occur when a student fails to maintain satisfactory academic progress, violates institutional policies, demonstrates unsafe conduct, engages in academic dishonesty, or otherwise fails to meet program requirements.

    Cheating, plagiarism, falsification of records, inappropriate conduct, harassment, threats, disruptive behavior, or violations of the Student Handbook may result in disciplinary action up to and including permanent dismissal.

     

    PAYMENT OPTIONS


    Students may select one of the following payment methods:

    □ Third-Party Sponsorship

    Payment Plan Requirements
    Unless the student elects to pay tuition in full or receives approved third-party funding, the student must satisfy all required enrollment obligations before beginning coursework.

    A minimum enrollment payment determined by the institution must be paid prior to course access being granted.

    Remaining balances must be paid according to the approved payment arrangement established with Pulse Institute of Allied Health.

    Failure to maintain payment arrangements may result in:

    • Suspension of online course access

    • Administrative withdrawal

    • Delayed graduation processing

    • Withholding of transcripts or certificates

    • Referral to collections when permitted by law


    REFUND & CANCELLATION POLICY


    The Registration Fee of $85 is non-refundable.

    Books, digital materials, software licenses, simulation access, training kits, supplies, uniforms, and educational resources become non-refundable once issued, accessed, activated, downloaded, shipped, or utilized.

    Students who withdraw from a program must submit written notification to the Institution.

    Refund calculations apply only to tuition and will be processed in accordance with Arkansas Division of Higher Education regulations and institutional refund policies.

    No refund shall be issued after a student has completed twenty-five percent (25%) or more of the program.

    Students acknowledge that course access, educational materials, and support services begin creating educational costs immediately upon enrollment.


    GRADUATION


    Students who successfully complete all academic requirements, satisfy competency requirements, meet attendance and satisfactory academic progress standards, and fulfill all financial obligations will be awarded a Certificate of Completion from Pulse Institute of Allied Health.

    The Institution reserves the right to academically dismiss any student who fails to demonstrate satisfactory academic progress, professional conduct, or compliance with institutional requirements.

    Completion of a program does not guarantee employment, certification, licensure, or job placement.

    Although placement assistance services may be available, Pulse Institute of Allied Health does not guarantee employment to any student or graduate.

    Replacement certificates, transcripts, books, supplies, and educational materials may be subject to additional fees.


    PRIVACY OF STUDENT RECORDS


    Student educational records are maintained and protected in accordance with the Family Educational Rights and Privacy Act (FERPA), applicable Arkansas laws, and institutional privacy policies.

    Educational records will not be released without proper authorization except as permitted by law.


    NOTICE TO GUARANTOR


    You have been asked to guarantee payment of tuition and fees for the student identified in this Enrollment Agreement.

    Before signing as a guarantor, carefully consider your financial responsibility. If the student fails to satisfy their financial obligations, you may be legally responsible for the unpaid balance.

    Pulse Institute of Allied Health may pursue collection efforts directly against the guarantor without first exhausting collection efforts against the student.

    Any default may affect the guarantor's creditworthiness and financial standing.

    By signing this agreement, the guarantor acknowledges understanding and acceptance of these responsibilities.


    COMPLAINT PROCEDURE


    Students are encouraged to first address concerns directly with the Institution.

    If a complaint cannot be resolved satisfactorily through institutional procedures, students may contact:

    ENTIRE AGREEMENT


    This Enrollment Agreement, together with the School Catalog, Student Handbook, Institutional Policies, Tuition Disclosure Statements, and any signed addenda, constitutes the entire agreement between Pulse Institute of Allied Health and the Student.

    No verbal representations, promises, statements, or understandings shall alter the terms of this Agreement unless reduced to writing and signed by authorized representatives of both parties.


    STUDENT CERTIFICATION


    By signing below, I certify that:

    • I have read and understand this Enrollment Agreement.

    • I have received access to or a copy of the School Catalog.

    • I understand tuition, fees, payment obligations, refund policies, and program requirements.

    • I understand that employment, certification, and licensure are not guaranteed.

    • I agree to comply with all institutional policies and procedures.
