Please note that each session is intended to be confidential and held in a smoke free safe environment to help provide a safe space for all attendees. Alcohol, Drug Paraphernalia, Abusive Behavior, Harassment of any kind, Fighting, and Loitering are Not Permitted/ Not Allowed on the premises of the Rt 9 Library & Innovation Center, the Rosehill Community Center, and Virtual Platforms. Health and Wellness Sessions are intended for educational purposes only and not intended to prescribe or treat any health or mental condition. If you have a chronic or any kind of health condition consult your treating physician prior to any stay active session that you plan to attend. All participants will be asked to sign a waiver prior to stay active sessions.

By signing this registration form you acknowledge that you have read and understood the above terms to attend The July 6th-10th Summer Wellness Week Session(s) and do not hold KF Health Consults, Well Women Empowerment, Fitness S'PORTER, The Garden Nurse Consulting Co, Staff, Partners, Rt. 9 Library Innovation Center, and the Rosehill Community Center Liable for any injuries, illness, accidents, incidents, and fatalities sustained or occur while attending the July 6th-10th Summer Wellness Week.
