    Purpose

    This survey supports the mission of the Connectional Lay Organization by assessing organizational effectiveness and strengthening leadership, accountability, ministry service, and organizational impact. Feedback will help identify strengths, opportunities for growth, and priorities for continuous improvement.

    Instructions

    Please complete this anonymous survey based on your observations and experiences with the Lay Organization at the Connectional level.

    Survey will take approximately 5 to 10 minutes to complete.

     

    Rating Scale
    1 – Strongly Disagree: Rarely demonstrates the behaviors or responsibilities described; significant improvement is needed.

    2 – Disagree: Demonstrates limited effectiveness and does not consistently meet expectations.

    3 – Neutral: Demonstrates moderate or inconsistent effectiveness; some expectations are met, while others need improvement.

    4 – Agree: Effectively fulfills responsibilities and demonstrates leadership, service, and commitment to the Lay Organization.

    5 – Strongly Agree: Consistently demonstrates outstanding effectiveness, modeling strong leadership, service, and alignment with the mission and values of the Lay Organization.
