Barnyard Olympics Registration Form

By participating in the Barnyard Olympics, I, the undersigned, hereby acknowledge and agree to the following: 1. Assumption of Risk: I understand that participation in physical activities and challenges involves inherent risks, including but not limited to slips, falls, muscle strains, and other injuries. I voluntarily choose to participate and accept all risks associated with the event. 2. Release of Liability: I, for myself and on behalf of my team members, heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless Pulse 712, the Sioux County Youth Fair, Sioux County Youth Fair Board Association, and the Iowa State University Sioux County Extension Office from any and all liability, claims, demands, or causes of action for any injury, loss, or damage to person or property arising out of or related to participation in this event. 3. Medical Treatment: In the event of an injury, I authorize event staff to seek emergency medical care and understand that I am responsible for any and all associated costs. 4. Responsibility for Damages: I accept full responsibility for any damages caused by myself or my team during participation in the Barnyard Olympics. 5. Photo/Video Release: I grant permission for Pulse 712 and event affiliates to use photographs and/or video taken during the event for promotional purposes. By signing below, I confirm that I have read, understood, and agree to the above terms.
