Axios Baseball around the clock registration form

Join Fr. Koby and the Core Team for a fun night at NBC baseball tournament "Baseball Around the Clock".

Schedule:
- 10 pm Candlelight Mass in Two Hearts Chapel (families welcome! This Mass fulfills Sunday obligation)
- After Mass (11 ish) head to the WSU stadium. Students will ride with chaperones.
- 11:30pm -? watch games at WSU. We will likely only see 2 games (12 am and 3 am).
- After the games, chaperones will bring students back to SEAS to watch baseball movies and have snacks in the Youth Room
- 7 am - pick up students.
COST: $25 will cover:
- Snacks we will bring into the stadium
- Snacks in the Youth Room when we get back
- Ticket covers all the games on Saturday 7/25 and Sunday 7/26. Games start at 12 pm on Saturday - 6 pm on Sunday. Games happen every 3 hours.
  - You are welcome to go to the earlier games if desired.
  - This does NOT cover any "baseball around the clock" promotions or check-in prizes (no shirt or prizes).
  - We purchased group tickets NOT baseball around the clock tickets.

Medical Authorization: I/We understand that the Catholic Diocese of Wichita and the Totus Tuus assume no responsibility for accidents which may occur in association with diocesan events and activities. I/We agree to use my/our personal insurance to cover any such incidents. I understand the possibility of unforeseen hazards and know the inherent possibility of risk. I agree to indemnify and hold harmless the Catholic Diocese of Wichita and/or the Office of Faith formation, its leaders, employees and volunteer staff from any claim arising from or in connection with attending this event and Code of Behavior

Code of Behavior: I agree to abide by and/or instruct Participant to abide by all rules and regulations as outlined by the aforementioned chaperones/representatives. I agree that if I/Participant fail(s) to abide in any way by the rules, that I/Participant can be dismissed from the trip/event and sent home immediately at my/Participant’s expense with no right of reimbursement or refund for any amount in connection therewith from the Catholic Diocese of Wichita or its chaperones/representatives.
