However, it is important to understand and acknowledge the following liability disclaimer:Assumption of RiskParticipation in dance classes, workshops, rehearsals, performances, and other activities offered by Passion for the Arts LLC is voluntary. Participants voluntarily assume all risks associated with their involvement in these activities.Dance and related activities involve inherent risks, including but not limited to physical exertion, potential for injury, and the risk of accidents. Participants acknowledge and accept these risks when engaging in dance-related activities.Participants are responsible for assessing their own physical condition and determining whether they are fit to participate in dance classes and activities. It is recommended that individuals consult with a healthcare professional before starting any new physical activity. By enrolling in our programs or participating in our activities, participants affirm that they have read and understood the risks associated with dance participation and have provided informed consent to participate.Participants, including students, parents/guardians, and visitors, hereby release Passion for the Arts LLC, its instructors, staff, volunteers, and affiliates from any and all liability for injuries, damages, or losses sustained during or as a result of participation in dance classes or related activities.Participants agree to indemnify and hold harmless Passion for the Arts LLC, its agents, and employees from any claims, demands, or actions arising out of or related to their participation in dance activities.Passion for the Arts LLC implements safety protocols and guidelines to minimize the risk of injury or accidents during classes, rehearsals, and performances. Medical Waiver: I hereby authorize the provision of any necessary medical care for my child/children in the event of an accidental injury, illness, emergency, or non-emergency situation until I can be reached. Furthermore, I accept responsibility for covering the costs associated with any required medical treatment. Tuition & Payment Policy: At Passion for the Arts Dance Center, we are dedicated to providing high-quality dance education while maintaining transparent and fair tuition and fee policies. Please review the following guidelines regarding tuition payments, fees, and related matters:1. Tuition Payments:Payment Schedule: Tuition fees are due on 5th. Payment schedules will be communicated to parents/guardians upon enrollment. Accepted Payment Methods: We accept payments via cash, credit/debit card, and online payment platforms. Please ensure that payments are made promptly to avoid late fees or class discontinuation. Late Payments: A late fee may be applied to tuition payments received after the due date. $20.00 Late Fee will be applied after the 15th of the month. Refunds: ALL FEES PAID TO PASSION FOR THE ARTS LLC ARE I acknowledge that tuition is non-refundable and will not be adjusted due to student absences or variations in the number of class weeks per month. Make-up classes are encouraged and will be announced by the studioNONREFUNDABLE Registration and Administrative Fees:Registration Fee: A one-time registration fee is required for each student upon initial enrollment. This fee includes dancers’ leotard and 2 pair of tights and secures the student’s place in the program. Annual Fees: An annual fee may be charged to cover expenses such as costumes and performance recital fees . I understand that recital fees and costume charges are billed separately from tuition, and statements for these fees will be provided during the course of the dance program The amount and purpose of the annual fee can be paid in installments and will be communicated to parents/guardians in advance. Sibling Discounts: Families with multiple children enrolled in our programs may be eligible for sibling discounts. The discount amount of 20% will be applied to up to 2 family members enrolled in the program. This excludes registration fee, recital fees, and costumes fees.

DROP OUT/MISSED CLASS POLICY: We understand that sometimes circumstances may require families to withdraw their child from the program. If a parent or guardian decides to withdraw their child from the dance program, the following guidelines apply: Notice of Withdrawal: Parents/guardians must provide written notice of their intent to withdraw their child at least [2] weeks in advance. Failure to do so will result in the obligation to pay for the upcoming month of classes. Refunds: No refunds will be issued for payments already made for the current term. Missed Classes Policy: If a student misses a class for any reason (including illness, vacation, or personal commitments), there will be no refunds or reduction in fees for the missed session. We encourage students to make up any missed classes during the same session, provided there is availability, but it is not guaranteed. Absences do not result in a reduction of the overall program fee

My dancer will be partcipating in PADC 3rd annual demonstration showcase (December 2026) and dance recital showcase (June 2027)

Costume Agreement: Ordering Costumes are ordered based on the dancer’s measurements provided at the time of fitting.Once an order is placed, all sales are final. Costumes cannot be canceled, returned, or refunded. 2. Payments; A non-refundable costume fee is required to secure your dancer’s costume. Parents may choose to pay in full or through installments (per studio payment schedule). Costumes must be paid in full by March 1st. Unpaid balances may prevent participation.3. .Exchanges due to sizing errors are not guaranteed and may result in additional charges and delays .4. Damages, Loss, & CareParents/guardians are responsible for proper care of costumes once distributed. Lost, stolen, or damaged costumes are the responsibility of the family, and replacement costs will apply if a reorder is possible. 5. Recital & Performances: All fees (including tuition, costume fees, and any other outstanding balances) must be fully paid before recital participation. Dancers must arrive with their costume(s) in clean and performance-ready condition. 6. Liability ReleaseI understand that PADC is not responsible for costume defects, delays from costume companies, or any additional costs beyond the original fee. Acknowledgment & AgreementI, the undersigned parent/guardian, have read, understand, and agree to the terms of the Costume Waiver & Agreement.

Parent/Guardian Agreement: I acknowledge that I have read, understand, and agree to all policies, procedures, terms, and conditions of Passion for the Arts LLC. I agree to fulfill my responsibilities as a parent/guardian throughout the dance season. I understand that all payments are final. Registration fees, tuition, costume fees, recital fees, and any other payments are non-refundable and non-transferable, regardless of withdrawal or dismissal. If I choose to withdraw my child, I understand that I will not receive a refund or credit and remain responsible for any outstanding balance. I understand that if my child's account reaches a past-due balance of $500 or more, my child may be placed on Financial Probation and may be restricted from participating in classes, rehearsals, performances, or other PADC activities until the balance is paid in full or an approved payment plan has been established with the Artistic Director. I understand that any unpaid balance remains my financial responsibility. Passion for the Arts LLC reserves the right to pursue collections or legal action, including seeking court costs, collection fees, and reasonable attorney's fees where permitted by law, to recover unpaid balances. By signing below, I certify that I have read, understand, and agree to all financial obligations and policies of Passion for the Arts LLC.
