I understand and accept that there are inherent and known risks of physical injury as the result of participation in such activities. In consideration of Old Hickory Church of the Nazarene's allowing the above named participant(s) to engage in these activities, I, on behalf of myself, any other parent/guardian and the participating, agree to release, discharge and indemnify Old Hickory Church of the Nazarene, its church board, employees, volunteers or any other church officials from liability for any and all claims, damages and injuries suffered by the participant in connection with participation in these activities or events.
