Youth Theatre Participant Agreement

To produce the most educational, rewarding, and professional rehearsal and performance experience possible for all participants, Central Florida Community Arts expects the following from each participating member of the Youth Theatre Programs:
- Attendance and Late Policies. Students are expected to arrive and leave rehearsals and performances promptly at the start and end times. If a student has not arrived within 5 minutes of their rehearsal or performance "call time" (the time the student is expected to arrive), we will call parents or guardians. If a parent/guardian is not present by 5 minutes past the stated rehearsal or performance ending time and an emergency has not been communicated to us, the family may be charged a $1 per minute babysitting fee after the five-minute grace period.
- Full disclosure of any conflicts with any program dates/times. Absences are taken extremely seriously, and any conflicts (absences, late arrivals, or early releases) must be properly communicated before your student’s audition (for auditioned programs) or by the program start date (for non-auditioned program) or your student may have their role, lines, or other responsibilities reassigned or, in serious cases, your student may need to be removed from their production. Refunds will not be issued if your student’s attendance, once a production has begun, affects their ability to take part in the program. Please note that conflicts with a tech/mandatory rehearsal or performance will automatically render your student ineligible to participate in a production at all.
- Appropriate behavior and responsibility at all times throughout the rehearsal and performance process. Participants in any Central Florida Community Arts program will be held to a standard of appropriate behavior and responsibility. Consequences for violations of this standard are as follows, and will be documented:
  - First violation: The participant will receive a private verbal warning.
  - Second violation: A parent/guardian of the participant will be notified
  - Third violation: CFCArts will contact a parent/guardian to set up a meeting between CFCArts staff, the participant, and a parent/guardian to discuss further steps. This may result in the student’s removal from the program or required transition to a virtual program, depending on the violation(s) in question.
  - Please note that if a participant commits an extreme violation (e.g. an extremeact of violence), one or more disciplinary steps may be skipped.
  - Violations include, but are not limited to: Absence or extreme lateness without prior approval or confirmed emergency; violence and/or bullying (physical,mental, or emotional); possession of an illegal item or substance; attendance while under the influence of an illegal substance; substantial and/or consistent lack of preparation or effort; highly and/or consistently disrespectful behavior towards program staff or other participants; vandalization; and theft.
Please note that students must be at least 4 years old and fully potty-trained by the date of their first rehearsal in order to participate.

Please read the following statements carefully.
By checking the box below, I do release and hereby agree to hold blameless Central Florida Community Arts and its board, staff, volunteers and agents from any and every claim arising, or which may be asserted by myself or my student by reason of participating in any activities associated with Central Florida Community Arts, whether arising out of negligent acts or omissions of any Releasee or otherwise. I also release the leaser/owner of properties on which any part of this Program is held.
By checking the box below, I do authorize CFCArts' staff or campus official, in the event my student is physically unable to agree, to give consent to a physician and/or hospital for emergency medical or surgical treatment while participating in this event. It is understood that I will assume any financial responsibility for any expense that may be incurred for said emergency treatment on behalf of my student.
In the event that any of the following symptoms are observed, by checking the box below, I authorize and understand that CFCArts’ staff may call emergency services to attend to my student at my own expense. If my student chooses to refuse treatment, CFCArts may not allow my student to rehearse or perform at a given event. Those symptoms are: Chest pain, shortness of breath, difficulty breathing, head trauma, neck or back trauma, loss of consciousness, non-superficial bleeding, undisclosed seizure conditions, burns, etc.

Check-In and Check-Out Procedures
- Check In: Students may check in no earlier than 15 minutes prior to their designated "call time" (communicated start time.)
- Check-Out: Starting immediately at the program’s designated end time, a person on the student’s approved pick-up list must check out the student.
- Students may not be checked out early unless there is an emergency cleared with staff, or approved early release.
- Photo ID: Any person checking out a student must be prepared to present a photo ID. Staff reserves the right to contact a parent/guardian until/unless photo ID is presented or the identity of the person checking out is proved beyond reasonable doubt as a safe adult approved by a legal parent/guardian.
- Approval List: Each student must submit an approval list (see below) with names, relationships, and phone numbers anyone permitted to check them out. Approval lists may contain underage persons such as an older sibling. To make a
  change/addition to an approval list, fill out the Student Information Change Form. (Please save this link for later!)
- Student Drivers: Students who are at least 16 years old and able and permitted to drive themselves to and from rehearsal or use a taxi or ridesharing service must have a waiver on file signed by the student and a legal parent/guardian (see below.) All other students under 18 must be checked out by a person on their approval list.
- Late Pick-Up: 5 minutes after the program’s designated end time, staff members will contact a parent/guardian if a student has not been picked up. If this is a persistent problem, we reserve the right to charge a babysitting fee of $1 per minute.

Student Driver/Rideshare Waiver

Students who are at least 16 years old are permitted to check themselves out at the end of their program, leave the facility by themselves, and drive themselves home. Students who are at least 16 years old are also permitted to alternatively use a ride share service, such as Uber or Lyft, after checking themselves out of rehearsal. CFCArts is no longer responsible for your child once they have checked themselves out. If your student is not at least 16 years old and/or does not have your permission to check themselves out of rehearsal, please select "NO" below with the reasoning that best fits your student.
