Please read our studio policies carefully before proceeding to the waiver. By booking an appointment, you agree to these operational terms.

• Cancellation & Rescheduling: Any cancellations or changes must be made at least 24 hours prior to your scheduled appointment. Cancellations made with less than 24 hours' notice will incur a fee equal to 50% of the scheduled service cost.
• No-Show Policy: Failure to arrive at your appointment without prior notice will result in a fee equal to 100% of the scheduled service cost, and you may be restricted from future bookings.
• Late Arrival Grace Period: I offer a strict 15-minute grace period. Arriving past this time will incur a $15 late fee added to your final bill. Your service time will be shortened to fit the remaining window, but you will still be charged full price.
• Automatic Cancellation: If you are more than 20 minutes late, your appointment is automatically canceled, treated as a "No-Show," and charged accordingly.
• No Extra Guests Policy: Due to limited space, safety regulations, and the focused nature of our detailed lash application, no extra guests, children, or pets are permitted in the studio during your session. Please arrive alone to your appointment.

 Informed Consent & Risks

I hereby consent to have eyelash extensions applied and/or removed by Kams Luxe Lashes. I understand that this procedure involves attaching synthetic extensions to my natural lashes using a professional-grade adhesive. I fully understand and accept that this cosmetic procedure carries inherent risks, including but not limited to eye irritation, redness, swelling, chemical burns to the cornea (if eyes are opened during the procedure), or temporary natural lash shedding.

Release of Liability

I explicitly agree to assume all risks associated with this procedure. I hereby release, acquit, and forever discharge Kams Luxe Lashes, its owners, employees, and independent contractors from any and all claims, demands, damages, or legal actions resulting from negligence or complications arising during or after the service. I understand that failure to follow the aftercare instructions provided to me may cause retention issues or adverse reactions, for which Kams Luxe Lashes is not liable.

Dispute Resolution (Arbitration)

In the event that a dispute arises from this service that cannot be settled amicably, I agree that the matter shall be resolved through binding, private arbitration rather than a traditional court trial. Arbitration will be handled by a mutually agreed-upon arbitrator, and the prevailing party shall be entitled to recover reasonable attorney fees and associated legal costs.
