✨For new and current clients✨ Please read carefully and fill out the form as thoroughly as possible. This form will close for new responses on Monday July 13th at 8pm. Responses may be shut off early if supply/demand is reached sooner. Emails will be sent out by late Monday evening through Tuesday the 14th to schedule your consultation time. ✨✨✨CHECK YOUR EMAILS/SPAM FOLDERS, and respond ASAP, as consultations will be scheduled virtually for July 15th, 16th, 17th and 18th. Actual Booking dates to do your tattoo will be determined during our virtual consultation, and will be for remaining spots in August/September through October, November, December 2026. Please read thoroughly before checking box: If your project is selected, you will be contacted via the email you’ve provided above to book a consultation. Be sure to respond to the email promptly to secure your spot. During your consult, we will discuss your ideas in further detail and set a time and date to do your tattoo. At the end of your consult, while we are still on the phone, I will have you send a $100 or $200 (depending on the size of your tattoo) nonrefundable deposit to hold your time slot. Deposits come off of the total price of your tattoo. If your project requires more than one session, then your deposit will come off of the final session of your tattoo. Deposits are to be sent through Venmo or Cashapp @Blackatlastattookc If you would prefer to pay with a debit or credit card instead please let me know. DEPOSITS ARE DUE AT THE TIME OF YOUR CONSULTATION. Please Do Not send your deposit until asked to do so during your consult.

CANCELLATION POLICY: If you need to reschedule for any reason, you must let me know 7 days before your appointment date, or your deposit will be forfeited. I send out confirmation texts about a week or two before your appointment to confirm the date, time and details of your design. Designs are completed AFTER I have received confirmation. If your phone number changes please let me know immediately so that I can update my records. If I do not hear back from you within 24 hours, I will attempt to contact you a second time. If I have not heard back within 48 hours of the original confirmation text to confirm, your appointment will be cancelled and your deposit forfeited. If your project requires multiple sessions and you need to reschedule more than one, even with notice, a new deposit may be required to book. A new deposit may be required to rebook for last minute reschedules. Deposits are put towards the agreed upon design that is discussed during your consultation. I will generally accommodate changes if discussed in a reasonable time frame prior to your appointment. However some requests I may not be able to accommodate, and your deposit remains nonrefundable if you make changes that I cannot do for any reason. Deposits and appointment dates are non transferable to other clients, family or friends. If you cancel your appointment date with the 7 days notice, and do not wish to reschedule at the time of cancellation, your deposit will not be refunded. I do understand that life happens, and will always do my best to accommodate within reason. However, these policies have been put in place to protect my time and effort as well as the time and effort of my other clients.
