1. Permission to Participate I, the undersigned parent/legal guardian, give permission for my child to participate in gym use, training, classes, and all related activities at Flinders University Sport & Fitness (FUSF). I understand that participation in physical activity carries inherent risks, and I acknowledge that my child must follow all FUSF rules, instructions, safety requirements, and behaviour expectations at all times.
2. Agreement to FUSF Policies I confirm that I have read and understood the FUSF Membership Terms & Conditions, Code of Conduct, and Direct Debit Agreement. I agree that my child will comply with all FUSF policies, and I accept responsibility for ensuring they understand and follow these requirements.
3. Direct Debit Membership Authorisation I acknowledge that a minor cannot enter into a membership contract, and therefore I am the contracting party for this membership. I authorise Flinders University Sport & Fitness (and its payment provider) to establish and maintain a direct debit membership on my behalf for my child’s access to the facility. I accept full responsibility for all membership fees, payment obligations, failed payment fees, and any changes or cancellations in accordance with the Direct Debit Agreement.
4. Medical & Emergency Consent I confirm that my child is medically fit to participate in physical activity. I authorise FUSF staff to provide first aid and, if required, to seek emergency medical assistance. I accept responsibility for any associated costs.
5. Parent/Guardian Declaration I declare that the information I have provided is true and correct, and I understand that this consent remains in place for the duration of my child’s membership unless revoked in writing.
