Acknowledgment of Risks

I, the Guardian, acknowledge that my child, the participant listed above, will be participating in the field trip to Knott’s Berry Farm organized by Saddleback Student Ministries Whittier. I am aware that participation in this trip involves certain inherent risks, including but not limited to physical injury, accidents, or property damage. I understand that these risks are associated with amusement park rides, walking, and other activities during the trip.

Assumption of Risk

I, the Guardian, voluntarily agree that my child may participate in this trip and hereby assume full responsibility for any risks, injuries, damages, or losses that may result from their participation. I understand that the church, its employees, and volunteers will make reasonable efforts to ensure the safety and well-being of my child; however, I acknowledge that accidents may still occur.

Medical Treatment

In the event of an emergency or injury during the trip, I authorize the trip organizers and/or emergency medical personnel to administer first aid and to seek medical treatment for my child if necessary. I understand that I will be notified as soon as possible in case of any emergency.

Release of Liability

I hereby release, discharge, and hold harmless Saddleback Church Whittier, its employees, volunteers and representatives from any claims, demands, actions, or causes of action for injuries, damages, or losses sustained by my child during this trip. This release applies to any claims arising out of or related to the activities and events during the trip to Knott's Berry Farm.

Photograph and Video Release

I grant permission for photographs or videos to be taken of my child during the trip and for these images to be used for Church purposes, including publications, social media, and promotional materials.

Agreement

By signing this form, I acknowledge that I have read and understood this waiver and release of liability, and I agree to the terms set forth above. I understand that by signing this document, I am waiving certain legal rights, including the right to sue.
