I understand as the parent/guardian for student named in this request that presenting a false document or false records for the purpose of school enrollment is an offense under TEXAS PENAL CODE §37.10, and a person who knowingly falsifies information on a form required for enrollment of a student in a school district is liable to the district under TEXAS EDUCATION CODE §25.001(h) for tuition and/or costs if the student is not eligible for enrollment in the district but is enrolled on the basis of the false information. I further understand that if the information provided in this request is ever found to be incorrect or untrue, that my student’s transfer status may be denied or revoked. I declare under penalty of perjury under the laws of this state that the information provided is true and correct, and of our own personal knowledge.

Grades: The student’s academic record or credit accumulation did not meet the district’s transfer criteria.
Attendance: The student’s prior attendance or tardy record did not meet the district’s standard for transfer students.
Discipline: The student’s prior disciplinary record does not comply with LaPoynor ISD transfer standards.
Grade Level Full: The requested grade level or campus has reached maximum enrollment capacity, and we are unable to accept outside transfers at this time.
