Please note that the session will take place on a concrete floor. In addition to bringing a yoga mat, please consider additional support if needed.
By signing this waiver, I agree and understand the following:
1. Water bottles must have a screw-top lid. Glass bottles are not permitted.
2. Participants must remain 3ft from the artwork at all times.
3. Food is not permitted.
4. Flash photography is not permitted.
5. In case instructors provide physical adjustments, I understand that I have the option to opt out by letting my instructor know my wish not to have physical adjustments.
6. I will notify staff if I do not want my photo taken.
7. I hereby release, waive, discharge, and hold harmless the institution, its directors, officers, staff, volunteers, affiliates, and partners from any and all liabilities arising from any untoward incident in my participation in any class, workshop, and relevant sessions which may result in injury, loss, damage, or death.
By signing this form, I hereby represent and warrant that I am physically fit and capable of participating in yoga classes. I agree and legally bind myself, with full understanding of the contents and meaning of the provisions above.
