Informed Consent and Acknowledgement
I hereby as the parent/guardian of the above listed minor, do herby authorize adult volunteers of Central Missionary Baptist Church (CMBC) as agents for the undersigned, to consent to any first aid required and any transport to emergent medical or surgical care deemed advisable by any accredited physician or surgeon in an approved emergency clinic or hospital.
I further release from liability CMBC and any of its ministers or leaders in the event of accident within route, during, or returning from the above mentioned event. This agreement does not apply to claims for intentional misconduct or gross negligence.
CONFIRMATION
BY ACKNOWLEDGING AND SIGNING BELOW, I AM DELIVERING AN ELECTRONIC SIGNATURE THAT WILL HAVE THE SAME EFFECT AS AN ORIGINAL MANUAL PAPER SIGNATURE.
THE ELECTRONIC SIGNATURE WILL BE EQUALLY AS BINDING AS AN ORIGINAL MANUAL PAPER SIGNATURE.
