The Northern Colorado Continuum of Care (NoCO CoC) is accepting project applications for the FY2026 U.S. Department of Housing and Urban Development (HUD) Continuum of Care (CoC) Program Notice of Funding Opportunity (NOFO).

This application is intended for organizations that have not participated yet in the NoCO CoC local competition during the winter 2025/2026 application cycle. Please complete this application in its entirety.

Please also include information on the proposed staffing structure (including any sub-recipients) to ensure successful project outcomes, the proposed length of period of performance (grant period), and a brief summary of other funding sources used for this project (e.g., in-kind donations, federal funding, other non-federal government funding). How to Calculate Length of Stay (LOS) Buckets for Transitional Housing (Unsuccessful Exits Only)
Follow the steps below to complete the table showing how many participants with unsuccessful exits fall into each LOS bucket:
- Less than or equal to 6 months (≤180 days)
- More than 6 months (>180 days)
1. List all clients who exited during the reporting period
  Include everyone whose enrollment ended between 10/1/2024 and 9/30/2025. Do not include participants who are still actively enrolled (no exit date).
2. Calculate Length of Stay (LOS) for each exited participant:
  1. Length of Stay = Exit Date – Entry Date (in days)
    This gives you the total number of days they spent in the Transitional Housing program.
3. Identify which participants had unsuccessful exits
  Only include clients who did NOT exit to permanent housing.
