PHOTOGRAPHY / VIDEOGRAPHY RELEASE
I understand and acknowledge that photographs, video recordings, audio recordings, and other media may be taken of children participating in church services, children’s ministry activities, camps, outreach events, and related programs sponsored or hosted by New Life Rehoboth Church Inc. and/or Faith Kingdom Ministries (“the Ministries”).
I grant permission for my child(ren)’s image, likeness, voice, and participation to be recorded and used by the Ministries without compensation for lawful ministry, educational, promotional, publicity, print, digital, social media, livestream, website, and other communication purposes, unless otherwise prohibited by law.
PARENT / LEGAL GUARDIAN CONSENT, RELEASE, AND LIABILITY WAIVER
As the parent and/or legal guardian of the child(ren) listed in this registration, I voluntarily give permission for my child(ren) to attend and participate in all activities, programs, services, classes, camps, transportation (if provided), outdoor events, recreational activities, and related ministry functions sponsored, organized, or hosted by New Life Rehoboth Church Inc. and/or Faith Kingdom Ministries.
I understand that participation in such activities may involve certain inherent risks, including but not limited to accidental injury, illness, allergic reactions, physical activity-related injuries, transportation-related incidents, and other unforeseen risks.
On behalf of myself, my child(ren), my heirs, executors, representatives, and assigns, I voluntarily assume all risks associated with participation and hereby release, waive, discharge, and hold harmless New Life Rehoboth Church Inc., Faith Kingdom Ministries, their pastors, directors, officers, employees, staff, volunteers, agents, representatives, and affiliated partners from any and all claims, demands, causes of action, liabilities, damages, losses, costs, or expenses (including attorney’s fees where permitted by law) arising from or related to my child(ren)’s participation, including but not limited to personal injury, sickness, property damage, or other losses, except where caused by gross negligence or willful misconduct where such limitation is required by applicable law.
I further agree to indemnify and defend the Ministries and their representatives against claims brought by or on behalf of my child(ren) or any third party arising from participation in these activities, to the fullest extent permitted by law.
I certify that my child(ren) are physically able to participate unless I have disclosed relevant medical conditions in writing. I authorize ministry staff or emergency personnel to obtain emergency medical treatment for my child(ren) if I cannot be reached, and I understand that I am financially responsible for any resulting medical expenses.
I acknowledge that ministry activities may involve reasonable supervision but that the Ministries cannot guarantee prevention of every accident, injury, illness, or incident.
AUTORIZACIÓN DE FOTOGRAFÍA / VIDEO / USO DE IMAGEN
Entiendo y acepto que durante los servicios religiosos, campamentos, clases infantiles, actividades comunitarias, eventos al aire libre y otros programas organizados por New Life Rehoboth Church Inc. y/o Faith Kingdom Ministries (en adelante, “los Ministerios”), se podrán tomar fotografías, videos, grabaciones de audio u otros medios visuales de mi(s) hijo(s).
Por medio de esta autorización, doy mi consentimiento para que la imagen, voz, nombre y participación de mi(s) hijo(s) puedan ser utilizadas por los Ministerios, sin compensación económica alguna, para fines ministeriales, educativos, informativos, promocionales, publicaciones impresas, redes sociales, transmisiones en vivo, sitio web y otros medios de comunicación permitidos por la ley.
CONSENTIMIENTO DEL PADRE / MADRE / TUTOR LEGAL Y EXENCIÓN DE RESPONSABILIDAD
Como padre, madre o tutor legal del(de los) menor(es) registrado(s), autorizo voluntariamente la participación de mi(s) hijo(s) en todas las actividades organizadas, patrocinadas o dirigidas por New Life Rehoboth Church Inc. y/o Faith Kingdom Ministries, incluyendo pero no limitándose a servicios religiosos, clases infantiles, campamentos, actividades recreativas, eventos comunitarios, actividades al aire libre y transporte, si fuese provisto.
Reconozco que toda actividad conlleva ciertos riesgos inherentes, incluyendo accidentes, lesiones, enfermedades, reacciones alérgicas, incidentes durante transporte, actividad física u otras situaciones imprevistas.
En nombre propio, de mi(s) hijo(s), familiares, herederos y representantes legales, acepto asumir voluntariamente dichos riesgos y libero, exonero y mantengo libres de responsabilidad a New Life Rehoboth Church Inc., Faith Kingdom Ministries, sus pastores, líderes, directores, empleados, voluntarios, representantes, colaboradores y ministerios asociados, de cualquier reclamación, demanda, pérdida, daño, lesión, enfermedad, gasto o responsabilidad legal que pudiera surgir como resultado de la participación de mi(s) hijo(s), excepto en los casos donde la ley aplicable determine negligencia grave o conducta intencional indebida.
Asimismo, acepto indemnizar y defender a los Ministerios y sus representantes contra cualquier reclamación presentada por terceros o en nombre de mi(s) hijo(s) relacionada con su participación, hasta donde la ley lo permita.
AUTORIZACIÓN MÉDICA DE EMERGENCIA
Certifico que mi(s) hijo(s) se encuentran en condiciones físicas adecuadas para participar, salvo cualquier condición médica previamente informada por escrito.
En caso de emergencia, y si no se me puede localizar de inmediato, autorizo a los líderes, personal ministerial o personal médico de emergencia a procurar el tratamiento médico necesario para mi(s) hijo(s). Entiendo y acepto que cualquier gasto médico, hospitalario o relacionado será mi responsabilidad como padre, madre o tutor legal.
SUPERVISIÓN Y RECONOCIMIENTO
Entiendo que los Ministerios proveerán supervisión razonable durante las actividades; sin embargo, reconozco que no es posible garantizar la prevención de todo accidente, lesión, enfermedad o incidente imprevisto.
Al firmar este documento, confirmo que he leído, comprendido y aceptado voluntariamente todos los términos de esta autorización, consentimiento, exención de responsabilidad y autorización médica.

By signing below, I acknowledge that I have read, understood, and voluntarily agree to the terms of this Photography Release, Consent, Liability Waiver, and Indemnification Agreement.
