Participant Waiver, Release of Liability

& Photo Consent Form

I acknowledge that participation in Grown Folks Field Day may include physical activities such as team games, running, jumping, tug-of-war, relay races, obstacle courses, water activities, slip-and-slide activities, and other recreational activities.

I understand these activities involve inherent risks including falls, slips, trips, strains, sprains, collisions, dehydration, and other injuries that may occur.

I voluntarily choose to participate and understand that I am responsible for assessing my own physical ability and limitations before participating.

I agree to participate safely, follow event rules, and respect event staff, volunteers, participants, and property.

In consideration for being allowed to participate, I voluntarily release and hold harmless Healing Roots Tribe, Better Together Society, event organizers, volunteers, staff, partners, vendors, and property owners from claims, demands, damages, injuries, losses, or liabilities arising from my participation, except where caused by gross negligence or intentional misconduct.

I understand that I participate at my own risk.

I confirm that I am physically able to participate in recreational activities included in this event.

I will notify organizers of any limitations, injuries, or concerns that may affect participation.

I understand hydration, breaks, and personal safety are my responsibility.

I understand alcohol consumption and physical activity may increase risk, and I agree to make responsible decisions.

I agree to treat all participants, staff, and guests with respect.

I agree to follow safety instructions and avoid reckless, aggressive, or unsafe behavior.

Organizers reserve the right to remove any participant whose behavior creates an unsafe or disruptive environment.

I understand photographs and videos may be taken during Grown Folks Field Day.

I grant Healing Roots Tribe and Better Together Society permission to use event photos/videos for promotional, marketing, social media, website, and future event purposes.

In the event of an emergency, I authorize event organizers to contact emergency services if necessary.

I understand I am responsible for any medical expenses incurred as a result of my participation.
