Due to some technical issues with our registration platform, this form is to gather all registration information for your selected Sydney FC Winter Holiday Clinic.

Upon completing this form, you will be sent a secure payment link to complete your transaction. Because of this, please ensure that the contact information you've provided is correct.

Sibling/Team Discounts are available when completing payment within a single transaction. Discounts are as followed (the discounts will be applied upon payment):

- 10% for two participants

- 15% for three participants

- 20% for four or more participants

Your registration will only be complete and further information provided once completing payment.
