Tank / LuLu Adoption Form

All About LuLu + Tank :
LuLu and Tank were surrendered to us due to having too much loose skin around their eyes. They've since had this tacked back and they've healed beautifully. They've both received multiple check ups with our vet and their eyes cause them absolutely no issues at all and they are completely healthy puppies.

If successful LuLu / Tank will need to be collected by the adopted from Swadlincote

I give my consent for Midlands Cleft Rescue to gather, process / store my personal data and use my personal data in decision making relating to adoption of a dog. I understand that I may withdraw my consent at any point.
The information I have supplied in this application is accurate to the best of my knowledge.
All pups leave fully health checked by our vet, with minimum first vaccinations, microchipped and a neutering contract in place. Adoption fee is £650.
