Driver Qualifications
- I possess a valid driver's license appropriate for operating the church vehicle.
- I am at least 25 years of age.
- I have informed SCBC of any restrictions, suspensions, or limitations on my driving privileges.
- I have not been convicted of reckless driving, driving under the influence (DUI/DWI), or any major traffic offense within the past 3 years, unless previously disclosed and approved by church leadership.
- I am physically and mentally capable of safely operating the vehicle.
- I will immediately notify church leadership if my driving status changes before or during the event.
Driver Responsibilities
I agree to:
- Operate the vehicle in a safe and responsible manner at all times.
- Obey all federal, state, and local traffic laws.
- Wear a seatbelt and require all passengers to wear seatbelts where available.
- Refrain from using a handheld mobile device while driving.
- Not operate the vehicle while under the influence of alcohol, illegal drugs, or any medication that may impair driving ability.
- Conduct a pre-trip inspection of the vehicle and report any safety concerns before departure.
- Ensure that passengers enter and exit the vehicle safely.
- Drive only approved routes and destinations related to the church event unless otherwise directed by church leadership.
- Report any accident, incident, citation, vehicle damage, or injury immediately to church leadership.
- Return the vehicle in a clean condition and with any required post-trip documentation completed.
Vehicle Use Policies
I understand and agree that:
- The church vehicle may only be used for authorized church activities.
- Unauthorized passengers are not permitted.
- Smoking, vaping, alcohol, illegal drugs, and weapons are prohibited in the vehicle except as otherwise authorized by law and church policy.
- Only approved drivers may operate the vehicle.
- The church reserves the right to revoke driving privileges at any time.
Accident and Emergency Procedures
In the event of an accident or emergency, I agree to:
- Prioritize the safety of all passengers.
- Contact emergency services when necessary.
- Notify church leadership as soon as reasonably possible.
- Cooperate fully with law enforcement, insurance representatives, and church officials.
- Complete any required accident or incident reports.
Assumption of Responsibility
I understand that operating a church vehicle is a significant responsibility. I agree to exercise reasonable care and judgment at all times while transporting passengers on behalf of SCBC.
I acknowledge that failure to comply with this agreement or church policies may result in suspension or termination of driving privileges.
Authorization
I certify that the information provided in this agreement is true and accurate to the best of my knowledge. I have read, understand, and agree to comply with the requirements outlined above.
