By registering for and participating in the Bollywood Dance Workshop hosted by YUVA Dance Academy, I acknowledge and agree to the following:

1. Acknowledgment of Risk

I understand that participation in this dance workshop involves physical activity and carries inherent risks, including but not limited to muscle strain, falls, injury, illness, or other physical complications.

I certify that I am voluntarily participating, am physically capable of taking part in the workshop, and accept full responsibility for my health and well-being. If I have any medical concerns or conditions, I understand that it is my responsibility to consult a physician before participating.

2. Assumption of Risk & Release of Liability

I voluntarily assume all risks associated with my participation in this workshop and release, waive, and hold harmless YUVA Dance Academy, the guest choreographer, instructors, staff, volunteers, venue partners, and affiliates from any claims, liabilities, damages, losses, or expenses arising from my participation, except where prohibited by applicable law.

3. Emergency Medical Authorization

In the event of a medical emergency, I authorize YUVA Dance Academy and its representatives to obtain emergency medical treatment on my behalf if necessary. I understand that I am solely responsible for any medical expenses incurred.

4. Photography & Media Release

I grant YUVA Dance Academy permission to photograph, film, and record my participation during the workshop. I understand that these photos and videos may be used for promotional, educational, marketing, website, and social media purposes without additional notice or compensation.

5. Registration & Refund Policy

• Registration is confirmed only after payment has been received and verified.
• Workshop fees are non-refundable and non-transferable unless the workshop is canceled by YUVA Dance Academy.
• YUVA Dance Academy reserves the right to make reasonable changes to the workshop schedule, venue, guest choreographer, or instructors if necessary.

6. Code of Conduct

To ensure a safe, welcoming, and enjoyable experience for everyone, I agree to:

• Treat the guest choreographer, instructors, staff, volunteers, venue personnel, and fellow participants with courtesy and respect.
• Follow all instructions provided by the instructors throughout the workshop.
• Arrive 10–15 minutes early for check-in.
• Wear comfortable dance attire and appropriate footwear suitable for movement.
• Inform the instructor of any injuries or medical conditions before participating.
• Respect the venue and its facilities and help maintain a clean environment.
• Refrain from disruptive, disrespectful, unsafe, or inappropriate behavior. YUVA Dance Academy reserves the right to remove any participant whose conduct compromises the safety or experience of others, without refund.

Participant Agreement

By submitting this registration form, I confirm that:

• I have read and understood this Waiver, Policies, and Code of Conduct.
• I voluntarily agree to all of the terms and conditions stated above.
• I certify that the information provided is accurate.
• I understand that my electronic acceptance is legally binding and has the same effect as my handwritten signature.
