I agree to provide proof my dog is current on their Distemper Combo (DHPP), Rabies, and Bordetella vaccinations. I understand that if I fail to provide records or my dog’s vaccines are expired that I will not be able to participate in any classes.

I understand that the $250 payment by credit card is required in full at the time of registration, and that once paid, the it will not be refunded for any reason, including should I decide to forgo the class. Furthermore, I understand that I will be automatically enrolled in membership renewal by credit card unless I cancel my membership prior to the renewal date.

TERMS AND CONDITIONS: This is a contract between Indi Edelburg (Dog Training By Indi) and the Owner identified below, wherein Owner is enrolling with dog in a one year training club membership.

TERMS OF PURCHASE: All payments are final and No Refunds shall be given should the owner choose to terminate the training at any point, including before, during, or after the introductory 4 week class, or at any point in the next 12 months following registration, including any missed sessions. The owner recognizes and agrees that they shall not be entitled to a refund of any amount under any circumstances.

AUTOMTIC RENEWAL: By completing and submitting this form and subsequent payment, you are agreeing to be automatically enrolled in a renewal payment arrangement. The membership will automatically renew for an additional period of 1 year (12 months) from the date of renewal. The payment will automatically be charged to the credit card on file at the start of each new membership term for a fee of $120, unless you submit a request to cancel your membership before the end of the then-current membership period applicable to you. If you do not wish to continue to be charged on a recurring basis, you must submit a written request to cancel the membership by contacting Indi Edelburg directly, before the end of the then-current membership period. If so cancelled, there shall be no refund of the then-current membership fee.

MISSED SESSIONS: Any request to reschedule a missed session is not guaranteed and is subject to additional charge.

HEALTH POLICY: The Owner certifies that their dog is current on all required vaccinations including Rabies, Bordatella, and Distemper Combo (DHPP). Furthermore, Owner agree to keep their dog’s vaccinations up-to-date and to submit proof of up to date vaccinations once yearly. Should Owner fail to keep their dog’s vaccines up to date, they forfeit the right to participate in class until the dog is properly vaccinated. No extension of membership shall be offered for missed sessions.

LIABILITY: If the dog causes any property damage, or bites or injures any animal or person, during or after training sessions, Indi Edelburg (Dog Training by Indi) and the Humane Society of Marathon County are not liable for any resulting losses and damages suffered or incurred, including any injury, sickness, or death, or any resulting claims, demands, lawsuits, costs or expenses, including attorney fees. The owner assumes full and complete responsibility for the dog's behavior during and after the training program. If the dog is injured during or after training sessions, Indi Edelburg (Dog Training by Indi) and the Humane Society of Marathon County are not liable for any resulting injuries, losses, damages, costs or expenses.
