    希望由圆景基金会（注册慈善机构 1209727）协助申请撤销罪名（declared void and set aside）并追回已缴罚金与费用。

    我授权圆景基金会（注册慈善机构 1209727）及其个案工作人员代表我办理本铁路逃票定罪事宜，包括：
    (a) 代表我与 HMCTS、治安法院及起诉的铁路公司联系；
    (b) 索取、接收并保管我的案卷与个人资料；
    (c) 以我的名义准备并提交撤销定罪（declared void and set aside）申请
    (d) 协助追讨和接受退款 (如果拿回退款,我愿意自愿捐赠35%到Circle Vision Foundation)

    圆景基金会将仅为协助你的个案而处理本表格中的个人资料与刑事定罪资料（criminal-offence data），依据 UK GDPR、《2018 年数据保护法》及本会隐私政策。我们安全保存，并仅在为你的个案所需时与法院、HMCTS 及铁路公司分享。
