Clone of Combo Exam Request

All HAPS Exams are delivered online. You have three choices for administering your HAPS Exam, all require a live proctor:

1. Student Owned Devices - Students will use their own device to take the exam in a classroom setting.
They will receive an invitation from ExamSoft with instructions on how to download the software to their device. They will only be able to download this software once on one device.

2. Computer Lab - The exam can be administered in a Computer Lab.
The Lab Manager will be provided with instructions on how to download the ExamSoft software to the devices. Please review the Examplify Lab Setup information. The Computer Lab option will be available starting August 3, 2026.

3. Loaner Devices - These are institution owned devices made available to students.
Usually the exam-takers are not afforded the appropriate administrative rights needed to download and Install Examplify. In which case, your device managers may need to collect these devices to perform the install or otherwise afford the needed administrative rights to the device stewards (exam-takers). The Loaner Device option will be available starting August 3, 2026.

There are two steps for ordering the HAPS Exam to be administered at your institution. First, we collect your information using this form. The information will be used to set up your exam account and schedule the exam for the specific date/time that you request. When you click the green submit button at the bottom of the form, you will be redirected to our web store to securely pay for the Exams online. We only sell the exam to institutions, and if you have not used the HAPS Exam before we will need to verify your position at your institution. The Exam team is located in the Eastern Time Zone. If you wish to receive support for technical issues, we advise scheduling your exam during this time period: starting at 7:30 AM Eastern to 8 PM Eastern. Outside of those times, the Exam team will be unavailable and you may receive delayed responses if you encounter any issues.

ExamSoft requires the Institutional Administrator (HAPS Exam Team) to upload all student rosters. To help ensure your exam is set-up in a timely fashion, please upload your student roster below using this template.

Lab User Column:
Y - If the user is taking the exam using the Lab or Loaner option
N - If you are using the student-owned device option

Non-Secure:
Y - If the user requires an accommodation that involves enabling a screen reader on their device.
N - If the user does not need this accommodation.

Extra Time:

Instructors may not take any HAPS exam or otherwise compromise the content of the exams. Instructors are responsible for adhering to security best practices as outlined in HAPS documentation.
Fee Structure:

1-14 exams $24.00 each
15-100 exams $17.50 each
100+ exams $15.00 each

The minimum order is 15 exams. If you need fewer than 15 exams because of small class size, please contact the HAPS office after you complete this form. Please see the FAQ on the website for more information. Orders of fewer than 15 exams are limited to one schuduled exam date/time and are $24.00 each.
When you hit submit, your exam request will be sent to the HAPS Business Office for processing. You will then be redirected to the HAPS online store to pay for your purchase. You must submit using the green button above for your request to be processed. You will receive email confirmation that your order was received and instructions for paying in ways other than the online store.
