Dove Dance Enrollment 2026-2027

This form will be used to officially enroll your dancer in the 2026–2027 Fall Dance Season at Dove Dance School and serves as both your registration and commitment to participate. By submitting this form and paying the annual enrollment fee, you are securing your dancer’s spot in their selected classes and acknowledging your commitment to the upcoming dance season.

The enrollment fee is $45.00 for the first dancer in the family and $20.00 for each additional dancer. This fee is non-refundable.

Please review all information carefully before submitting, as this form will serve as the primary record for your dancer’s enrollment for the 2026–2027 season.

Liability Waiver & ReleaseI understand that participation in dance classes, rehearsals, performances, and related activities during the 2026–2027 Dance Season involves physical activity and carries an inherent risk of injury. I voluntarily assume all risks associated with my dancer’s participation.By signing this form, I agree to release, waive, and hold harmless Dove Dance School, its owners, directors, instructors, staff, volunteers, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any injury, illness, loss, or damage that may occur as a result of participation in the 2026–2027 Dance Season, except in cases of gross negligence or willful misconduct.I acknowledge that I have read and understand this waiver and agree to its terms as a condition of my dancer's participation in Dove Dance School's 2026–2027 Dance Season.

In the case of an emergency, what hospital would you want your dancer taken to?

I understand that I am financially responsible for the tuition associated with the class(es) and enrollment slot(s) reserved for my dancer during the 2026–2027 Dance Season at Dove Dance School. I understand that tuition is charged monthly and reserves my dancer’s place in class regardless of attendance. Tuition is a fixed rate and is not prorated, refunded, or adjusted based on missed classes, holidays, school breaks, vacations, illnesses, or other absences. I understand that I may withdraw my dancer from classes without further financial obligation by providing written notice to Dove Dance School on or before September 25, 2026, or within four (4) weeks of my dancer's first trial class, whichever is later. After the applicable withdrawal deadline has passed, I understand that I may choose to withdraw my dancer from classes; however, I will remain financially responsible for the tuition associated with my dancer’s reserved enrollment slot for the remainder of the 2026–2027 Dance Season. Tuition will continue to be billed and payment will remain due regardless of attendance or withdrawal after the withdrawal deadline. By signing below, I acknowledge that I have read, understand, and agree to these financial policies and commitments for the 2026–2027 Dance Season.
