1. ASSUMPTION OF RISK
I understand that participation in dance, cheer, and related athletic activities (the “Activities”) offered by Game Day Ready Dance LLC involves inherent risks, including but not limited to falls, collisions, muscle strains, sprains, fractures, illness, permanent disability, and death. I voluntarily assume all risks associated with participation in the Activities, whether known or unknown, for myself and/or the minor participant named below.
2. RELEASE OF LIABILITY
To the fullest extent permitted under the laws of the Commonwealth of Pennsylvania, I, on behalf of myself, my heirs, and (if applicable) the minor participant named below, release, waive, discharge, and hold harmless Game Day Ready Dance LLC, its owners, coaches, instructors, guest recruiters, volunteers, and staff, along with Radnor Township Activity Center and any co-hosting studios, sponsors, or affiliated organizations, and their respective officers, directors, employees, agents, and representatives (collectively, the “Released Parties”), from any and all claims, demands, causes of action, damages, losses, or liabilities arising from or related to participation in the Activities, including claims arising from the ordinary negligence of the Released Parties. This release does not apply to injury caused by gross negligence or intentional misconduct.
3. MEDICAL TREATMENT AUTHORIZATION
In the event of an injury or medical emergency, I authorize Game Day Ready Dance LLC staff and event personnel to secure emergency medical treatment for the participant if a parent or guardian cannot be immediately reached. I understand that I am responsible for any medical expenses incurred as a result of such treatment.
4. HEALTH ACKNOWLEDGMENT
I certify that the participant is physically capable of safely participating in the Activities and has no condition that would prevent participation or create an unreasonable risk to the participant or others. I agree to disclose any relevant medical conditions, allergies, or physical limitations to Game Day Ready Dance LLC staff prior to the start of the event.
5. PHOTO AND VIDEO RELEASE
I grant Game Day Ready Dance LLC and its representatives permission to photograph, film, record, and otherwise capture the participant's likeness, voice, and participation during the event. I authorize the use of such materials for promotional, educational, marketing, social media, website, and advertising purposes, without compensation or further approval.
6. CODE OF CONDUCT
I understand that all participants are expected to conduct themselves respectfully and professionally toward staff, instructors, and fellow participants. Game Day Ready Dance LLC reserves the right to remove any participant whose behavior is disruptive, unsafe, inappropriate, or detrimental to the event environment, without refund.
7. GOVERNING LAW
This agreement shall be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania. If any provision of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.
ACKNOWLEDGMENT AND SIGNATURE
By signing below, I acknowledge that I have read this entire agreement, understand its terms, and voluntarily agree to be bound by it. If the participant is under 18, this agreement must be signed by a parent or legal guardian on the participant's behalf.
