I, the undersigned parent or legal guardian, give permission for my child to participate in the Mahabah, Inc. youth retreat and related activities, including overnight camping, hiking, swimming, boating, outdoor recreation, group activities, transportation, and other activities associated with the retreat.Acknowledgment and Assumption of RiskI understand that participation in an overnight camping retreat involves inherent and potential risks, including but not limited to falls, slips, injuries, illness, insect or animal bites, water-related accidents, boating or swimming-related incidents, weather conditions, physical exertion, outdoor hazards, transportation-related risks, and other unforeseen circumstances.I understand that these risks cannot always be eliminated, even with appropriate supervision and reasonable safety precautions. I voluntarily allow my child to participate and accept these inherent risks on my child's behalf.Release and Waiver of LiabilityTo the fullest extent permitted by applicable law, I hereby release and agree not to hold liable Mahabah, Inc., its board members, mentors, volunteers, chaperones, representatives, and individuals assisting with the retreat for claims, demands, damages, losses, or expenses arising out of or related to my child's participation in the retreat, including claims arising from the ordinary negligence of the released parties, except to the extent such release is prohibited by law.I understand that this waiver does not release liability for conduct that cannot legally be released under applicable law.
